Social Services

Social Services is now called Community and Adult Care. This department works in partnership with individuals, groups, communities and other organisations (including the National Health Service) to protect older people. There is an adult services Helpdesk with trained customer service officers to help you search for information.

Community and Adult Care (Your Circle) has a range of people working to provide this:

  • A social worker – assesses and plans the support needed by people at home. They will either arrange the support package or organise a direct or indirect payment for you to purchase your own choice of care.
  • A care manager – usually a social worker but may be someone like a district nurse, an occupational therapist or someone from a home care organisation performing the same role.
  • An occupational therapist –provides rehabilitation at home using daily activities and can advise on and arrange adaptations to the house to enable safe living.

This team can organise a range of services for people with dementia who live at home. This could include:

  • Personal care – assistance with washing and dressing etc.
  • Preparation of meals
  • Assistance with shopping and pension collection
  • Arranging respite care
  • Arranging residential and nursing care
  • Support following discharge from hospital for a short period

Community Teams

Multi-disciplinary, multi-agency health and social care teams offer specialist assessment,treatment and care specifically to older adults with dementia and their carers, in their own homes and the community. Most service users are referred by their GP and other health or social care professionals but referrals can also be made by carers and the service user themselves. These are the teams for Gloucestershire. The teams consist of Social Workers, Community Psychiatric Nurses and Occupational Therapists.

Occupational Therapists

A person with dementia is entitled to a specialist assessment (for assistive equipment and adaptations), by an occupational therapist (OT).

An occupational therapist (OT) can visit the home to assess whether items such as handrails, grab rails or level access showers/baths would be helpful. Most people find the OT's visit and subsequent adaptations very helpful.
